Does depression manifest differently in women and men?

Women and men have different ways of seeking help. While depressed women seek help at clinics, depressed men, considering expressing psychological complaints taboo and seeking treatment a sign of weakness, are reluctant to seek psychiatric help and are more likely to try to treat their depression with alcohol, fighting, going to jail, or suicide.

Symptoms of depression are similar in men and women, but men express their symptoms differently. This is primarily due to the patriarchal mindset, stemming from upbringing, that a man must always be strong. Men raised with the motto "men don't cry" feel compelled to be strong in all circumstances, finding it difficult even to admit to themselves when they are experiencing difficulties. Because expressing emotions is culturally considered a feminine trait, and because of myths about being controlled, successful, and strong, men tend to hide their feelings. While trying to suppress feelings of depression, they experience restlessness and tension. Since anger, aggression, rudeness, and rage are culturally considered more masculine behaviors, men with depression display these symptoms more readily.

Can depression occur in children and adolescents?

In children, school phobia, school refusal, and excessive clinging to parents can be signs of depression. In adolescents, poor academic performance, substance abuse, antisocial behavior, sexual seduction, school absenteeism, and running away from home can be signs of depression.

How is depression diagnosed in old age? Is it common?

Many stressors confront individuals as they age. Retirement brings a disruption to a established daily routine, potential social isolation, decreased income, and a loss of the meaning derived from productive work. Failure to adapt to these changes can damage self-esteem and lead to depression. Furthermore, the deaths of family members and close friends, the emergence of significant health problems, and financial difficulties, coupled with a lack of adequate support, can all contribute to late-life depression. Studies report that 25-50% of older adults experience depressive symptoms.

Depression in old age is not a natural consequence of aging. It can be difficult for families to recognize depression in their elderly relatives. Because the emotional expression of grief in the elderly is limited, and complaints are more often expressed through physical illnesses or cognitive impairments (e.g., forgetfulness, inattention), it can often be misinterpreted as dementia or a direct effect of old age. Older individuals may also choose not to speak out for fear of upsetting those around them, or they may avoid seeking psychiatric help for fear of being perceived as a weakness.

The elderly family member's participation in family activities, sleep patterns, appetite, frequency of crying, and presence of distress can provide clues. Increased physical or cognitive complaints in the elderly should always raise suspicion of depression. If symptoms of emotional distress in the elderly are persistent and do not lessen with vacation or conversation, depression should also be suspected. If depression that begins in old age is not recognized and treated, it can last for years. Studies have shown that depression in the elderly is not adequately diagnosed and treated by general practitioners.

Can depression cause sexual dysfunction or impairment of the function of the genitals?

Depression leads to decreased interest in sexuality and impaired sexual performance in both men and women. In fact, in latent depression, sometimes the first symptom may even be a lack of sexual desire. In men, erectile dysfunction (impaired erection), and in women, difficulty with lubrication (arousal), and orgasm problems can occur. Women may experience irregular or temporary cessation of menstrual periods during depression. Men often don't admit that depression affects their sexual performance. They usually perceive fluctuations in sexual performance as a failure of their masculinity. In reality, the problem is simply a symptom of depression, which is a medical condition. Changes in reproductive function can also be observed.

What is the likelihood of suicide in depression?

Suicidal thoughts and actions occur in 20-40% of individuals with mood disorders. The majority of those who commit suicide have previously attempted suicide. Unfortunately, the risk of suicide is not always directly related to the severity of symptoms. It cannot be generalized that it occurs only in severe depression and not in mild depression.

The likelihood of suicide is twice as high in older adults as in younger adults. The suicide attempt rate among hospitalized patients is approximately 15%. 10-15% of patients with severe and recurrent depression die by suicide. 40-70% of patients with depression have suicidal thoughts. 70% of all suicides are related to depression. The idea that those who talk about their suicidal thoughts are less likely to commit suicide is incorrect. Half of suicidal individuals discuss their suicidal intentions with a friend or relative within 24 hours before exhibiting suicidal behavior.
