Everything About Attention Deficit Hyperactivity Disorder (ADHD)

Do I have ADHD?

Which of the common experiences associated with ADHD do you have?

  • missing or overlooked details

  • careless mistakes

  • Difficulty focusing during long activities such as attending lectures.

  • inability to continue listening

  • easily and constantly distracted

  • The concept and management of bad times

  • inability to organize

  • misplacing items

  • forgetfulness

  • to feel restless

  • feeling of unease

  • The need to move even in a place where one should be quiet and motionless.

  • talking too much

  • impatience

  • interrupting people while they are speaking or completing their sentences

How is ADHD diagnosed?

Identifying the distinguishing signs and symptoms of ADHD in adults can be challenging. No single test can confirm the diagnosis. To make a diagnosis:

  • A physical examination and some laboratory tests are necessary to help rule out other possible causes of your symptoms .

  • Information is collected by asking you questions about current medical issues, your personal and family medical history, and the history of your symptoms.

  • ADHD diagnostic and rating scales or psychological tests, which can help us gather and evaluate information about your symptoms, may be administered to you and family members such as your spouse and parents.

When should you consult a doctor?

If any of the listed symptoms consistently disrupt your life and cause you distress, you may want to consult a mental health professional to determine if you have ADHD. Only psychiatrists can diagnose and oversee treatment for ADHD.

Other Psychiatric Disorders Commonly Seen in People with ADHD

Comorbid mental disorders are common in adults with ADHD. It is estimated that 13% of adults with ADHD experience comorbid depression, and 25% experience comorbid anxiety disorder.

1. Co-occurring mental health conditions (substance use disorder, severe depression, anxiety, etc.) can make diagnosing ADHD more difficult.
2. Co-occurring mental disorders (e.g., borderline personality disorder) can mimic ADHD and lead to misdiagnosis.
3. They can lead to ADHD being overlooked because it is often only referred for as a comorbid diagnosis.
4. Missed ADHD diagnoses can complicate the treatment of co-occurring mental disorders, and often present with a treatment-resistant appearance due to untreated ADHD.

Unfortunately, we see that ADHD is often overlooked in applications, and individuals with ADHD are incorrectly or incompletely diagnosed with borderline personality disorder or even bipolar disorder. Besides the labeling, the inability to find the truly correct treatment can create despair in individuals with ADHD. Even if there is a co-occurring personality disorder, the chaotic life and negativity experienced due to untreated ADHD can exacerbate the symptoms of the co-occurring disorder and lead to it being classified as resistant or severe by someone who is not an expert in ADHD.

In adulthood, individuals often seek professional help due to anxiety, depression, or even burnout syndrome that often accompany ADHD. Because they describe anxiety or depressive symptoms as complaints, the ADHD diagnosis may remain undetected unless the specialist also suspects and inquires about these symptoms. Therefore, a thorough psychiatric examination is essential.

Anxiety Disorder Associated with ADHD

25-50% of individuals with ADHD have a comorbid anxiety disorder. 31% of individuals with ADHD also meet the criteria for Generalized Anxiety Disorder (GAD). While anxiety disorders are seen in 5-15% of children, this rate increases to 15-35% in children with ADHD. 27% of children with ADHD have more than one anxiety disorder. Agoraphobia, simple phobias, separation anxiety disorder, social phobia, and OCD are more common in children with ADHD.

There is a complex and powerful relationship between ADHD and anxiety. Estimates suggest that 50% of people with ADHD experience anxiety at some point in their lives. The risk for almost all anxiety disorders is higher in people with ADHD. Furthermore, ADHD and anxiety share similar symptoms, which can lead to diagnostic confusion, especially when both are present.

The inattentive type of ADHD appears to have higher rates of comorbid anxiety, which can present a particularly complex clinical picture. Anxiety accompanying inattentive ADHD is the most difficult to differentiate diagnostically. Without hyperactivity, it can be especially challenging for clinicians to distinguish whether inattentiveness stems from anxiety or from difficulty regulating attention due to ADHD. Therefore, individuals with both anxiety and ADHD are often diagnosed later than those without anxiety issues.

Another challenge is that ADHD is often accompanied by secondary anxiety (anxiety related to ADHD difficulties), unlike generalized anxiety disorder. Secondary ADHD anxiety involves worrying about difficulties with executive functions (worrying about school and work, missing details, stress stemming from difficulties managing schedules, time, and tasks, etc.).

While anxiety secondary to ADHD is often linked to executive functions (finance, health, relationships, school, and work), generalized anxiety disorder accompanying ADHD is more general and pervasive. Individuals with generalized anxiety experience uncontrollable worry most days; this worry is disproportionate to the actual stressor.

If anxiety is also present, this can worsen ADHD symptoms. Anxiety can curb impulsivity and hyperactive behavior but can exacerbate inattention. Individuals with ADHD who have anxiety disorders tend to have more severe anxiety symptoms, an earlier age of anxiety onset, and more comorbid mental health diagnoses and substance use problems compared to individuals without ADHD who have anxiety disorders. The presence of anxiety may also predict a poorer response to stimulant medications used in ADHD treatment.

Am I diagnosed with ADHD or Borderline Personality Disorder?

IPD and BPD can be similar in some ways. Common characteristics between the two conditions include:

  • Impulsivity: Impulsive behaviors are seen in both BPD and ADHD. Making sudden decisions and engaging in risky behaviors are frequently observed in both diagnoses.

  • Emotional instability: Sudden emotional fluctuations, prominently seen in BPD, can also occur in some individuals with ADHD. However, in ADHD, this is not as intense or persistent as in BPD.

  • Relationship difficulties: Individuals with both diagnoses may experience difficulties maintaining relationships. However, the causes and manifestations of these difficulties differ.

There are significant differences between BPD and ADHD, and understanding these differences is crucial for an accurate diagnosis:

  • Emotional intensity: In BPD, emotional instability is excessively intense and prolonged. In ADHD, emotional fluctuations may occur, but they are shorter in duration and less intense.

  • Relationships: In IPD, relationships are based on a lack of trust and fear of abandonment, whereas in ADHD, relationship problems are more related to attention deficit or inability to focus.

  • Attention and focus: Attention and focus problems are prominent in ADHD, but these symptoms are not as prominent in BPD. Emotional and relational problems are more dominant in BPD.

  • Behavioral and psychological origins: While BPD is often linked to emotional injuries or traumas from childhood, ADHD has more of a neurodevelopmental basis.

Bunun yerine hareket temelli meditasyon deneyebilirsiniz.

Is ADHD a treatable disorder?

Some individuals with ADHD may experience fewer symptoms as they age, but some adults continue to experience significant symptoms that interfere with daily functioning.

Therapies that focus on better ways to monitor your behavior can be very helpful. In addition to individual therapy, group therapies or community activities can also be beneficial in improving communication and developing problem-solving skills.

Family members learning about ADHD will significantly improve their relationship with the individual who has ADHD.

What is the mechanism of action of medications used in ADHD treatment?

People with ADHD tend to have lower-than-expected activity in certain brain regions. These affected regions regulate communication between other brain regions, giving you a range of abilities known as "executive functions." These include planning, reasoning, decision-making, and your ability to direct and focus your attention. In ADHD, this is referred to as "executive dysfunction." While not as dominant as dopamine and norepinephrine, altered levels of the neurotransmitters serotonin, glutamate, and GABA also play a role in the symptoms of ADHD.

Dopamine (DA) is a neurotransmitter found in abundance primarily in brain regions involved in reward circuits, learning and memory, and locomotor activity signaling. These regions contain specific D1 and D2 subtype receptors for these functions. First, I'd like to discuss how neurotransmitters work in the nervous system. Neurotransmitters are chemical messengers involved in nerve transmission, and their function is to transmit a specific message from one nerve cell to another. For DA, the function is to transmit the reward signal. In the synapse, the area where two adjacent cells meet and messaging occurs, the first cell releases dopamine, and the second cell attaches to the dopamine via receptors, receiving the message. For controlled messaging, there are also transporters (DATs) in the synaptic cleft that reabsorb excess dopamine.

However, in patients with ADHD, the concentration of dopamine transporter (DAT), which is responsible for the reuptake of DA into presynaptic neurons, is higher. This can cause dopamine levels in the synaptic cleft to decrease before they can exert their effect. Therefore, ADHD is essentially not a dopamine deficiency, but rather an excessive and rapid clearance of dopamine from the synaptic cleft. Medications used to treat ADHD (e.g., methylphenidate, amphetamine salts, and bupropion) act by inhibiting dopamine reuptake, meaning they do not add excess dopamine to the system externally.

Norepinephrine (NE) is the neurotransmitter responsible for performance in tasks requiring sustained attention. In ADHD, norepinephrine transporter (NET) activity is reduced in the frontoparietal-thalamic-cerebellar regions. Methylphenidate, amphetamine salts, and atomoxetine exert their effects by inhibiting NET. Furthermore, methylphenidate exerts its effects on working memory via α2-receptors, where NE binds, while improving sustained attention via α1-receptors.

Guanfacine and clonidine are other ADHD medications that can be used as second-line treatment in resistant cases or as primary treatment depending on the underlying medical condition. Both guanfacine and clonidine exert their effects via α-receptors to which neutrophils bind.

Another neurotransmitter whose role in ADHD is often unknown is serotonin. Low serotonin levels in the blood have been found to be associated with ADHD. Furthermore, serotonin levels in the cerebrospinal fluid show a positive correlation with hyperactivity symptoms. Therefore, some antidepressants that affect the serotonergic system are also considered second-line alternatives in the treatment of ADHD.

Increased glutamatergic activity in the frontal and striatal brain and elevated glutamate levels in the anterior cingulate cortex are associated with increased excitability in ADHD. Furthermore, GABA levels are decreased in the anterior cingulate, somatosensory, and motor cortices in ADHD, which is associated with impaired response inhibition.

Neuroimaging studies, such as MRI, reveal differences in brain structure and functionality, particularly between children with ADHD and neurotypical children. Children with ADHD have been reported to have smaller total intracranial volumes and smaller subcortical brain regions (nucleus accumbens, amygdala, caudate, hippocampus, and putamen), as well as reduced cortical thickness and volume (especially in the frontal, cingulate, and temporal regions, and the fusiform gyrus). Functional Magnetic Resonance Imaging (fMRI) studies primarily observe changes in connectivity within multiple brain networks related to executive functions, attention, emotions, sensory, and motor activities.

How can I cope with ADHD?

Because ADHD is a complex disorder and every person is unique, it's difficult to make recommendations for all adults with ADHD. However, some of the suggestions listed below may help:

  • At the start of each day, make a list of the tasks you want to complete that day. Prioritize them.

  • Make sure you don't try to do too much or everything at once. Break down tasks into smaller, more manageable steps.

  • Leave yourself reminder notes; you can use Post-it notes for this. You can put reminders on the refrigerator, bathroom mirror, in the car, or other places where you'll see them.

  • You can keep an appointment book or an electronic calendar on your phone/tablet to track appointments and deadlines.

  • You can carry a small notebook or an electronic device with you to jot down your thoughts or things you need to remember.

  • Following a consistent daily routine can help you avoid losing items like your keys or wallet.

  • You can ask for help from your family members or other loved ones.

How can psychotherapy help me with ADHD?

Common types of psychotherapy for ADHD include:

  • Cognitive Behavioral Therapy (CBT) teaches specific skills to manage your behavior and replace negative thought patterns with positive ones. It can also help you cope with life challenges such as school, work, or relationship problems, and address other mental health conditions like depression or substance abuse.

  • Marriage Counseling and Family Therapy: This type of therapy can help loved ones cope with the stress of living with someone who has ADHD and learn what they can do to help. This type of counseling can improve communication and problem-solving skills.

In Adult ADHD Psychotherapy:

  • Learn how to reduce your impulsive behavior.

  • Develop better problem-solving skills.

  • Begin to come to terms with past academic, work, or social failures.

  • Improve your time management and organizational skills.

  • Improve your self-esteem.

  • Learn ways to improve your relationships with family, colleagues, and friends.

  • Develop strategies to control your anger.
